Customer Service ID Card section duty hours are 0700 – 1600. Appointments are 0715 thru 1515 with no appointments from 1100 – 1300.


Customer Service is primarily an appointment based office. To schedule an appointment please call 424-4075 or 424-5604.


Emergency walk-in appointments are available to active duty military members, Department of Defense civilians, and contractors with ID card-related work stoppage concerns.  Those hours are from 0700 to 1400.

For ID Cards
Two forms of ID are required for a sponsor, spouse and dependent children over the age of 18 to obtain a new ID card.

For lost/stolen ID cards, SrA and below must present a lost/stolen ID card letter indorsed by their first sergeant.

If the sponsor is deployed, civilian spouses must have a Power of Attorney to get a new ID Card unless they possess the sponsor’s deployment order or a DD Form 1172 signed by the sponsor and it is dated within 90 days.

Dependents (18 years or older) do not have to have the sponsor present if they possess a DD Form 1172 signed by the sponsor within the last 90 days.

Students who wish to have a dependent ID card made/re-issued, need a signed DD Form 1172 from the sponsor and a letter from their school registrar--this letter must be on the current school letterhead, stating the student will be enrolled for the next term, the student is full-time, and be signed by the school registrar.

The average wait time is heavily influenced by the length of time it takes to make an ID card which is approximately 15-20 minutes for a CAC and 7-10 minutes for family member/retiree cards.

Defense Enrollment Eligibility Reporting System (DEERS)
Report family status changes to Customer Service ID Card section to ensure that DEERS is updated. 

Name Changes
original/certified copies of marriage certificate stating the new name, certificate/divorce decree (in English), or court order are required.

To enroll a spouse in DEERS the following is required:
U.S. citizens, original or certified copies of the marriage certificate, photo ID, and an original social security card or a receipt from the Social Security Administration showing they have ordered a social security card with the married name.
For non-U.S. Citizens, the original or certified copies of marriage and birth certificates (translated in English) and 2 forms of identification.

To enroll a newborn baby into DEERS
An original or certified copy of the birth certificate (in English), certificate (proof) of Live Birth authenticated by the attending physician or other responsible person from a U.S. hospital or medical treatment facility is required.

To enroll stepchildren into DEERS
An original or certified copy of the birth certificate and the original social security card is required.
DEERS is a DoD-wide eligibility system, so we can take care of you and or your family
even if you are not at your base of assignment.
If you are on leave or TDY and lose your ID card, you can obtain a new card at any DoD installation with DEERS/PIV CAC capability.
